Typhoid fever is caused by S. typhi, a Gram-negative bacterium. procedure, Widal Test (also referred as Weil-Felix Test) is used to facilitate the diagnosis.
Principle of Widal test Patients infected with Salmonella produce antibodies against the antigens of the organism. Stool culture-positive after the 4 months indicates carrier state. Urine culture is positive during the second to the third week in 25% of the cases, even if the blood culture is negative. Widal test is positive after 7 to 10 days of infection.

214: 2192). We report here four cases of nontyphoid salmonella infections where Widal test titres to S. typhi H and 0 antigens are significantly.
Widal Test is an agglutination test which detects the presence of serum agglutinins (H and O) in patients serum with typhoid and paratyphoid fever. When facilities for culturing are not available, the Widal test is the reliable and can be of value in the diagnosis of typhoid fevers in endemic areas.

This test was devised by Frank Widal in 1896. Widal originally described the test to diagnose Salmonella enterica serotype Paratyphi B infection. The Widal test result is positive: If the TO antigen titre is greater than 1:160 in an infection that is active, OR If TH antigen titre is greater than 1:160 in older infection or in immunized individuals
The Widal test may be falsely positive in patients who have had previous vaccination or infection with S. Typhi. Besides cross-reactivity with other Salmonella species, the test cannot distinguish between a current infection and a previous infection or vaccination against typhoid. Widal test is an agglutination test in which specific typhoid fever antibodies are detected by mixing the patient’s serum with killed bacterial suspension of Salmonella. These bacteria suspension carry specific O, H, AH and BH antigens and after mixture, they are observed for clumping (antigen-antibody reaction). Positive Widal test (antibody titre against S. typhi O antigen of 1:160) was recorded in 61.2 per cent of patients with bacteriologically confirmed typhoid fever and in 58.8 per cent with culture negative but clinically suggestive typhoid fever.

The Widal test is a test procedure that involves mixing of the bacteria which cause typhoid fever to the blood consisting of certain antibodies taken from an infected person. It is an assumptive serological test used for undulant fever or enteric fever.
There are an increase and the peak in the third to the fifth week of infection. After that, the titer starts to fall for several weeks. There are no universal standards that define the cutoff dilution of agglutinating antibodies to indicate a positive Widal test. The very low specificity of the assay (50% to 70%) and the inability to discern active from previous infection or vaccination means that the assay should rarely, if ever, be used. The Widal test is reported by giving the titer for both O and H antibodies.
